Stalk Symbiote

This tendril of flesh harvested from a living aberration can be embedded into the skull of an aberrant horror. To attune to this item, you must stitch it onto your scalp and leave it for the entire attunement period, during which the stalk burrows into your skull to bond with your brain. While attuned to this item, you have advantage on Wisdom (Perception) checks. In addition, you and any of your allies within 30 feet of you can't be surprised, except when incapacitated by something other than nonmagical sleep. The stalk can't be removed while you're attuned to it, and you can't voluntarily end your attunement to it. If the stalk is sliced from your body, it grows back in 1d4 hours.
